We achieve this through increased community engagement, strengthened partnerships, improved systems, and better delivery of existing policies, programs, and resources.
Diversity, equity, and inclusion are at the core of everything we do. Perhaps our biggest challenge yet, we are devoted to driving systemic change to allow all people to live their healthiest, fullest life. It’s bigger than healthcare access- we focus on upstream factors where the barriers begin. This includes things like access to healthy foods, equitable jobs, safe housing, and social justice to name a few.
With goals as big as ours, it’s important that we focus on the areas where we can bring the most impact. We are increasing access to:
Community-centered: All our work begins with hearing from the priority populations we are working to support. We’ll never start a solution without talking to those we are proposing it can help. We have to understand the challenges and perspectives of those we are serving throughout any work we do.
Aligned resources and partners: We can’t do this work alone, and we know we aren’t. Many organizations have the same priorities we do. We work to remove barriers to collaboration, and strengthen our network by bringing in financial and programmatic resources.
Built on what works: Research and implementation efforts have happened in other communities. We don’t need to reinvent the wheel- we need to enhance, improve, and tailor it to meet our community needs and assets.
Measurable: Our work starts with the end in mind- what outcomes do we hope to achieve, and how will we get there? We carefully map our how we will monitor and measure our impact from the beginning. This allows us to adapt our strategies to make sure we achieve what we set out to and lets us build more replicable for future communities.
We rely on our partners to help define local need, expand our reach, and advocate for change. We work with many partners, including other nonprofits, health systems, government agencies, schools and colleges, corporate sponsors, and foundations, to name a few.
